If re-encoding is requested (e.g., to change the bitrate), FFmpeg decodes the AAC and re-encodes it at the target bitrate using the native AAC encoder or libfdk_aac.
When using stream copy (the default), audio quality is identical to the original — no generation loss occurs. If re-encoding to a lower bitrate, expect minor quality reduction. iTunes M4V files typically contain AAC at 128-256 kbps, which is already high quality for most listening scenarios.
